Please join us for our annual FREE Sports Physicals at TriState Family Practice & Internal Medicine, Clarkston. NO APPOINTMENT NECESSARY! Details: Monday, June 22 | 4:00pm - 6:00pm TriState Family Practice & Internal Medicine 1119 Highland Avenue, Suite 10, Clarkston, WA Look for the signs and balloons! Please bring the required physical form from the school your child attends or will be attending. Be sure to have the demographics (name, birth date, address) completed before you arrive. Generic forms will be available. Parents must be present with children under 18 years old
